Plot[]
A man and his dog go hiking through West Virginia and is unfortunate enough to bear witness to an organ harvesting operation being conducted by town physician Doc Fraser and members of the Wallace family. After he is discovered, the hiker becomes their next target, and Doc Fraser removes his kidney. Addle-brained Bunt Wallace is responsible for disposing of the body. The hiker is still alive however and remains so just long enough to stumble out into the road where he is discovered by motorist Molly Sue Allen. The man dies, but Molly Sue's intervention leads to the discovery of numerous rotting corpses that have either been taken out into the swamp or stacked up inside of a secluded cabin. One of the bodies is that of her own child. Driven by sorrow, rage, and a need for revenge, Molly Sue convinces her friend Richie, ex-husband Oliver and Oliver's new girlfriend Ellie Johnson to seek out the old witch Haggis. They become part of a conjoined ritual to summon the demon of vengeance known as Pumpkinhead. Pumpkinhead seeks out those who have been marked by Haggis' curse, but this type of covenant comes at a price, and with each victim that Pumpkinhead takes, the summoners experience the pain of the kill as if it were their own. When Doc Fraser learns the truth about Pumpkinhead, he surmises that he can bring the threat of the demon to an end by killing those that summoned him.
