Mateo was a native of South America and the captain of a boat that took a film crew led by Terri Flores and snake hunter Paul Serone down the Amazon River. Mateo was attacked by a giant anaconda that coiled around him and crushed him to death.

Mateo was a veterinarian assistant who was employed by Cecilia Pederson to pose as an anesthesiologist during John Kramer's experimental medical procedure to cure his cancer in Mexico City. Mateo was also stealing medications from his job to sell to Gabriela, a drug addict who was also employed by Cecilia to pose as a caretaker. After John learns that the whole operation was a scam when he returns to give a gift to Cecilia, Mateo is abducted by Amanda Young after John learns his whereabouts from Mark Hoffman. Mateo is later placed in the brain surgery trap where he has to perform a self-craniotomy in order to dissolve a piece of his brain tissue in an enzyme that will release a key. Mateo manages to retrieve a piece of brain tissue but it fails to dissolve in time, causing a hot-iron mask to close over Mateo's face, burning him to death.
